As a result of ageing, the number of Europeans affected by Parkinson’s, Alzheimer’s (and other forms of dementia), and Cardiovascular disease is rising. Moreover, many of these patients face other chronic conditions.
It is within this context that TeNDER will develop and pilot an integrated health care model. The project aims to improve the quality of life of patients and those that surround them. First, by facilitating communication between social and health care professionals, and extending the autonomy of patients that can live independently. Second, by making TeNDER’s model for integrated care fit for widespread implementation, to benefit patients beyond project years.
TeNDER partners include user organisations, Small Medium Enterprises (SMEs), academic institutions and policy experts. Located in Spain, Portugal, Italy, Slovenia, Greece, Germany and Belgium, our consortium reflects how EU-funded research can cut across disciplines, sectors and country borders to benefit societies.
