TeNDER

A multi-sectoral project working to develop an integrated care model
to manage multi-morbidity in patients with neurodegenerative diseases

Scenario

An Integrated Health Care model for patients with neurodegenerative conditions

As a result of ageing, the number of Europeans affected by Parkinson’s, Alzheimer’s and other forms of dementia is rising. Moreover, many of these patients face other chronic conditions, including cardiovascular diseases, and diabetes, among others.

It is within this context that TeNDER will develop and pilot an integrated health care model. The project aims to improve the quality of life of patients and those that surround them. First, by facilitating communication between social and health care professionals, and extending the autonomy of patients that can live independently. Second, by making TeNDER’s model for integrated care fit for widespread implementation, to benefit patients beyond project years.
